Heroes Around Me - 2018-2019 Reflections

CONTEST INFORMATION FOR WEST HAVEN ELEMENTARY 
& SAND RIDGE JUNIOR HIGH

The Reflections program is a National program that encourages students to express themselves through their artistic talents.  This program is for all students (preschool - 12th grade).
Students may enter one piece in each of 6 categories:

Visual arts 
Literature
Photography
Musical Composition
Film
Dance

If you use a picture of someone else, or their voice you need to get their consent (read the guidelines to decide what you actually need consent for.

Students compete in different age groups:
Primary (K - 2nd grade) 
Intermediate (3rd - 5th grade)
Middle / Junior (6th - 8th grade) 
High School (9th-12th)


The top two entries from each category and age group move onto the Council level (at Roy High).

REMEMBER students can enter in multiple categories but only ONE time per category.  (Example, 1 visual arts, 1 musical composition, 1 photography).

Forms you need to enter Reflections

1 - Student Entry Form

     Click on the highlighted text, print (use the print button) and fill out the form.

2.  Chose the Artist Statement Form for your category, print and fill out the form 
 ALL ENTRIES MUST HAVE AN ARTIST STATEMENT FORM*

3.  If someone else is shown or their voice is used in your entry you need a consent form signed by their parent/guardian (or if an adult they need to sign it themselves)

*Younger children can dictate their forms to an adult and if a student competing in the special needs category is unable to communicate their statement it may be omitted.

NEW forms for 2013-2014 Reflections Program

Theme:  Believe, Dream Inspire
What do you believe in? What are your dreams? Who inspires you? Choreograph a dance, produce a
film, write a poem or story, compose a song, take a picture or create a piece of visual art or 3 dimensional
art and tell us your interpretation of the theme: “Believe, Dream, Inspire.” Pick up a PTA Reflections Entry
Form today from the white binder in the office and return it by October 8th.

Time to start thinking about your reflections project . . . 

This year's theme is:  The Magic of a Moment . . .

These two (three?) are required with every entry

1)  2012-2013 Official Student Entry Form  
                    *This form is missing Photography so if you print from here be sure to write that in if your entry is a photography entry.  I will have corrected forms available in the office at the school if you'd rather just grab one there.
   

2)   2012-2013 Artist Statement Form.pdf

 This is the form where you explain how your entry relates to the theme, it really helps the judges, it can be as short as a few words or as long as 250 words.

3 ) UTAH MEDIA RELEASE FORM - This form has been required in the past but is not yet available so I don't know if it will be used this year . . . I hope to have the answer before school starts.

   - EDIT 9/19/2012, I still have not heard definitively if this form is required, but copies are available in the office. -

This one may be needed, if you use someone else's image or voice

2012-2013 Reflections Consent Form

Required when someone's voice or image can be recognized and is included in the entry

 (someone other than the artist).
